- If you possess an NQF 6 Diploma/Degree (post-school qualification), state it. Specify the type of degree, diploma, or vocational certificate.

- Answer a series of yes/no questions pertaining to your citizenship, physical or mental disabilities, criminal or departmental offenses, tattoos, previous public service employment, and involvement in outside businesses or conflicts of interest.
- For some questions, additional details are required, such as specifying the type of disability, detailing a criminal offense, indicating the position of a tattoo, or explaining an outside business or conflict of interest.
- Declaration:
- Read and understand the declaration which encompasses the limitations of available posts, selection processes, and consent for checks and screenings. It also captures the acknowledgment that undergoing the selection processes doesn’t guarantee an appointment.
- Fill in the date, place, and then affix your signature to authenticate the information you’ve provided.
