Role Overview
The Rehabilitation Research Assistant position involves conducting follow-up assessments with patients who have recently been discharged from Tokai Estate. The primary responsibility of the assistant will be to administer the Modified Barthel Index (MBI) to evaluate the functional outcomes of these patients. This data is essential in tracking progress and providing insights into the rehabilitation process.
The successful candidate will be expected to engage with patients and caregivers to gather critical information on their recovery, including any falls, hospital readmissions, or complications experienced post-discharge. Key Responsibilities
The Rehabilitation Research Assistant will have several key responsibilities:
- Patient Follow-Up:
The primary duty will be to conduct follow-up calls with patients approximately one month after discharge. These calls will assess progress, gather feedback on their rehabilitation journey, and document responses to standardized questions related to their recovery. - Data Collection, Documentation, and Auditing:
The assistant will accurately record MBI scores and responses from patients. Additionally, the assistant will be responsible for maintaining a well-organized database and ensuring data integrity. - Patient Support and Escalation:
