This is a fantastic opportunity for individuals looking to contribute to the field of healthcare, specifically in the area of rehabilitation and patient recovery. The role is a half-day, 6-month contract, offering flexible hybrid work arrangements.

About Faircape Health and Tokai Estate

Faircape Health operates the Tokai Estate Sub Acute Care and Rehabilitation Centre, which is renowned for providing restorative care and rehabilitation services for patients recovering from various medical conditions. These conditions include neurological, orthopedic, surgical, and respite care. The facility focuses primarily on acute conditions that significantly affect the rehabilitation process. At Faircape Health, the goal is to bridge the gap between hospital care and home recovery.

The facility prides itself on offering world-class care, supported by a Multidisciplinary Team (MDT) of healthcare professionals who work together to provide the best therapeutic environment for healing. The emphasis is on improving patient outcomes in all aspects of rehabilitation.

    Role Overview

The Rehabilitation Research Assistant position involves conducting follow-up assessments with patients who have recently been discharged from Tokai Estate. The primary responsibility of the assistant will be to administer the Modified Barthel Index (MBI) to evaluate the functional outcomes of these patients. This data is essential in tracking progress and providing insights into the rehabilitation process.

The successful candidate will be expected to engage with patients and caregivers to gather critical information on their recovery, including any falls, hospital readmissions, or complications experienced post-discharge. This role supports research initiatives aimed at enhancing rehabilitation programs and monitoring the effectiveness of the care provided.

The Rehabilitation Research Assistant will have several key responsibilities:

    1. Patient Follow-Up:

    2. Data Collection, Documentation, and Auditing:
The assistant will accurately record MBI scores and responses from patients. Additionally, the assistant will be responsible for maintaining a well-organized database and ensuring data integrity.
    3. Patient Support and Escalation:
If any concerns or issues arise during the follow-up calls, the assistant will document these findings and escalate any complex cases to the clinical team for further intervention.
    4. Reporting and Insights:
Monthly summaries of follow-up findings will need to be compiled. These summaries will highlight trends and patterns in patient outcomes, which are vital for continuous improvement in rehabilitation services.
    5. Administrative Support:
The assistant will assist in scheduling follow-up calls, ensuring accurate patient contact details, and maintaining records. They will also support the team in preparing reports and research documentation.
