    Job Summary: Clinical Pharmacist Vacancy

    The Clinical Pharmacist Vacancy is a permanent position that involves providing clinical pharmaceutical services in alignment with hospital policies, standards, and regulations. As a Clinical Pharmacist, you will play a vital role in ensuring the safe and effective use of medications, optimizing patient outcomes, and collaborating with the healthcare team to deliver comprehensive care.

    Key Responsibilities:

    • Lead assigned patient service lines and therapeutic programs, ensuring compliance with clinical pharmacy standards and regulations.
    • Collaborate with the Group Clinical Pharmacist to implement medication utilization frameworks, standards, and systems at the hospital.
    • Facilitate pharmaceutical care services, direct patient care programs, medication utilization systems, and therapeutic care areas.
    • Monitor and review patient profiles and medication charts to identify, prevent, and mitigate drug-related problems, including improper medication use, dose selection errors, over-prescription, medication without indication, and treatment failures.
    • Implement and manage stewardship activities and surveillance programs aimed at improving medication safety and effectiveness.
    • Track and evaluate pharmacy programs at the facility for quality assurance and benchmark them against both local and international best practices.
    • Collaborate with healthcare providers within the hospital to integrate clinical pharmacy services into the overall healthcare delivery system.
    • Work within established protocols and frameworks for medication use and control.
    • Identify high-risk patients and implement appropriate interventions to enhance quality and safety.
    • Make evidence-based, patient-centered medication recommendations to optimize therapy outcomes.
    • Provide comprehensive pharmaceutical services throughout the hospital, ensuring accurate, safe, and timely medication therapy based on individual patient needs.
    • Ensure all clinical pharmacy transactions are accurately recorded and reported on approved platforms and systems.
    • Follow established escalation processes related to clinical pharmacy and provide relevant recommendations that consider cost-effective initiatives adopted within the group.

    Minimum Qualifications and Experience:

    • Bachelor of Pharmacy (B.Pharm) and/or MSc (Med) Pharm or M. Pharm (Clinical Pharmacy).
    • 5 – 7 years of experience in a clinical pharmacy environment.
    • Hospital experience is mandatory, with relevant operational exposure and practical application of clinical pharmacy knowledge in a hospital setting.
    • Proven track record of implementing clinical pharmacy programs and interventions.

    Key Competencies:

    • Extensive knowledge of clinical pharmacy practices and hospital pharmacy operations.
    • Strong leadership skills with the ability to manage patient service lines and therapeutic programs effectively.
    • Proficiency in analyzing patient profiles and medication charts for clinical interventions.
    • Ability to implement and monitor stewardship and surveillance programs.
    • Strong collaboration and communication skills to work effectively with multidisciplinary healthcare teams.
    • Attention to detail and the ability to handle multiple priorities in a high-pressure environment.
    • Competency in using clinical pharmacy platforms and ensuring accurate data recording.
